Carpal Tunnel Relief Without Surgery: What Actually Works

Numbness that wakes you at 3 a.m., a thumb that fumbles keys, a hand that feels swollen but is not — carpal tunnel syndrome has a recognizable pattern, and mild to moderate cases often respond well to conservative therapy.
Why symptoms are worst at night
Most people sleep with their wrists curled. Flexion narrows the carpal tunnel and raises pressure on the median nerve, which is why tingling peaks in the middle of the night and eases when you shake the hand out.
A custom night orthosis holds the wrist in neutral so the nerve is decompressed for the eight hours you are not thinking about it. For many patients this single change produces the first real night of sleep in months.
The conservative program, step by step
- Custom-molded neutral wrist orthosis for sleep, adjusted for fit and comfort.
- Median nerve and tendon gliding exercises to restore the nerve's ability to move within the tunnel.
- Ergonomic and activity modification for keyboard, phone, tool and steering-wheel positions.
- Edema control and postural work through the neck, shoulder and forearm, where the nerve can also be irritated.

Signs you should be evaluated sooner

- Many patients notice better sleep within the first two weeks of wearing a properly fitted night orthosis. Strength and dexterity changes take longer and are tracked with measurements at each reassessment.
- Are over-the-counter wrist braces good enough?
- Off-the-shelf braces are often built with the wrist in slight extension and rarely fit a specific hand well. An orthosis molded to your wrist in neutral is more comfortable and more effective overnight.
