What to look for when you arrive
Look for the “Hand Rehabilitation — Deborah Austin, OTR, CHT” sign above Suite 106. Accessible parking is directly in front of the door, and the entrance is step-free.

Before your appointment
Download the intake packet, complete it at home, and bring it with your photo ID, insurance card and physician referral. Completing it in advance means more of your first visit goes to treatment.
