Our commitment
Hand Rehabilitation Specialist, Inc. is required by the Health Insurance Portability and Accountability Act (HIPAA) to maintain the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we use your information
- Treatment: to plan and deliver hand therapy, fabricate orthotics, and coordinate care with your referring physician or surgeon.
- Payment: to verify benefits and bill your insurance carrier or you.
- Health care operations: for quality review, scheduling, and internal administration of the practice.
- As required by law: including public health, workers' compensation, and legal proceedings.
Any other use or disclosure — including most marketing and any sale of PHI — requires your written authorization, which you may revoke at any time in writing.
Your rights
- Inspect and receive a copy of your records.
- Request an amendment to information you believe is incorrect.
- Request an accounting of certain disclosures.
- Request restrictions on uses and disclosures.
- Request confidential communications at an alternate address or phone number.
- Receive notice of a breach of unsecured protected health information.
- Receive a paper copy of this notice on request.
Website privacy
This website does not include a patient portal and does not collect health information online. Intake forms are downloaded, completed on paper, and brought to your appointment. Please do not send protected health information by email or web form.

To exercise a right or file a complaint, contact our Privacy Officer at 561-362-8757. You may also file a complaint with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
