Custom Orthosis vs. Drugstore Brace: What You Are Really Paying For

A twenty-dollar brace from the pharmacy is sometimes exactly right. Other times it is the reason someone spends six months not getting better.
What a custom orthosis does differently
- It positions the specific joint at the specific angle your diagnosis calls for, not an average angle.
- It immobilizes only what must be immobilized, so the rest of the hand keeps working.
- It is molded in the office to your hand and remolded as swelling and motion change.
- Pressure points can be relieved on the spot instead of tolerated for weeks.
When an off-the-shelf brace is fine
General soreness, short-term support during a heavy task, or a mild strain with no positioning requirement can often be handled with a commercial brace. If it is comfortable and your symptoms are improving, it is doing its job.
When it usually is not enough
- Post-surgical positioning after tendon, nerve, joint or fracture procedures.
- Trigger finger, where a single joint must be blocked and the others free.
- Thumb CMC arthritis, which needs stability at the base and freedom at the tip.
- Nerve compression at night, which needs true neutral wrist positioning.
- Mallet finger and other injuries where a few degrees of position decide the outcome.
